Children under Sixteen

Children under Sixteen

REQUIREMENTS FOR CHILDREN UNDER SIXTEEN:

Both parents must execute the application for a child under sixteen. The child him/herself must also be physically present for the application. In addition to the items required for five-year passports listed above, the following additional items are required for a child under sixteen:

  1. Original or certified true copy of the child's U.S. birth certificate or Consular Report of Birth Abroad
  2. Valid photo ID for both parents with name in English (such as a US, Taiwan or other passport or US driver's license) that matches the names on the child's birth certificate or Consular Report of Birth Abroad

When applying for a child whose previous passport photograph was taken before age two, parents must bring a series of "growing photos" of the child beginning with baby photos and continuing through the child's current age.  Photos showing the child with the parents are also helpful.
If one parent cannot appear to execute the application, the parent who appears must also bring the following items:

  1. Signed, notarized consent from the absent parent giving permission to issue a passport to the child
  2. The absent parent's valid photo ID (or a certified copy) with name in English that matches the name on the child's birth certificate or Consular Report of Birth Abroad

Special Circumstances:

  • Divorce:  If the parents are divorced, the applying parent must provide a court order granting custody of the child to the applying parent.
  • Death:  If one parent is deceased, the applying parent must bring the original or certified copy of the death certificate.  If both parents are deceased, the guardian must provide both death certificates and/or proof of guardianship.
  • Whereabouts unknown:  If the location of one parent is unknown, the applying parent must bring a written statement in English noting the date of last contact with the absent parent, efforts made to locate the parent, and any other relevant information.  A Consular officer will make a determination on a case-by-case basis.  More evidence may be required.
