Lost or Stolen Passports

You must appear in person to report a lost or stolen passport and apply for a replacement.  Click here to make an appointment in Kaohsiung.

If you have an emergency, you may apply for a limited validity emergency passport which will be issued while you wait.  In case of an emergency, if there are no appointments available at the AIT Kaohsiung Branch Office before your scheduled departure, you may call AIT Kaohsiung Branch Office at (07)238-7744 to apply for an emergency passport. You may contact our office during the normal business hours by calling (07) 238-7744 or call (02) 2162-2319 emergency line for after hours emergencies. Please note that the emergency line is only for non-visa emergency services. We will attempt to assist you as expeditiously as possible. However, applicants with appointments have priority.

The following items are required:

  1. Written report of lost or stolen passport from the nearest National Immigration Agency Office (http://www.immigration.gov.tw).  (Required to depart Taiwan.)
  2. Two 2” x 2” (5cm x 5cm) color passport photos (head size 1” to 1 3/8”) with a white background, taken within the past 6 months. For details about the photo size, please click here.
  3. Valid photo identification (in English) as proof of identity
  4. Birth certificate, naturalization certificate, or expired U.S. passport as proof of U.S. citizenship, or certified copies of these items.  (A regular photocopy of the missing passport is not proof of citizenship, but may be useful to research your citizenship.)
  5. Completed application form (DS-11) and lost or stolen form (DS-64)  
  6. US$135 fee for adults and US$105 for minors.  There may also be a US$150 fee for a search of the passport files.  All fees are payable by cash (NT$) or with a major credit card.   

After you have received a replacement passport from AIT, you must return to the nearest National Immigration Agency office where you filed the initial lost/stolen passport report to apply for an Exit Permit. An Exit Permit is required in order to leave Taiwan using this new passport.

Your passport will be ready in approximately two weeks and will be sent to you via registered mail.
